Remember our normal cancellation policy still applies to appointments booked online.

Life happens! If you need to cancel or reschedule an appointment, we require a minimum of 48-business hours’ notice for evaluations and follow-up appointments. We require 72-business hours’ notice for cancelling or rescheduling Ketamine/Spravato treatments. (Weekends not included). ie. If your appointment is on a Monday, the cancellation/rescheduling must be made by the same hour on the preceding Thursday for Ketamine/Spravato treatments or preceding Friday for all other appointment types.

Arriving 15 minutes or more past the start time of your appointment is considered a failure to show for services and is subject to the cancellation fee explained below and the client will need to be rescheduled. Clients who consistently cancel late or do not show up for appointments will be discharged from our services. (3 or more times in one service year).

If you cancel/reschedule with less than 48-business hours’ business notice or fail to arrive on time to a Follow-Up appointment your card on file will be charged a $100 fee. Evaluation appointments incur a $150 cancellation fee.

If you cancel/reschedule with less than 72-business hours’notice or fail to arrive on time to a Ketamine Treatment appointment your card on file will be charged $25 less than the current price of a ketamine treatment listed on our website. Spravato appointments will be charged $225.

Amy is a psychiatric nurse practitionerwho completed her Doctorate of Nursing Practice at the University of Utah. She has worked as a mental health nurse for the last 10 years. She spent 3 years working on an inpatient adolescent unit in Utah county and then worked another 3 years in an outpatient clinic. Most recently, she lived in New York City and worked with the chronically homeless. She has extensive experience working with individuals experiencing depression, anxiety, ADHD, women’s health and trauma.

Along with medication management, Amy is passionate about therapy and is a trained trauma therapist. She is specialized in EMDR and has been practicing for almost 3 years. She strives to provide a safe space for her clients to tell their story so they can find meaning, empowerment, and peace. Her goal is to help untether individuals from mindsets, emotions, or sensations that may be impeding them from experiencing their most fulfilled life.

Melissa is a psychiatric nurse practitioner with a Doctorate of Nursing Practice from the University of Utah. She graduated from the University of Utah with her Bachelor’s in nursing and has worked as a registered nurse in a residential eating disorder treatment center and as a pediatric oncology nurse.

She has 6 years of mental health experience as a psychiatric technician and registered nurse caring for people with diverse diagnoses and backgrounds. She has extensive experience working with individuals with eating disorders, depression, anxiety, OCD, PTSD, mood disorders, and ADHD. Melissa is HAES® aligned and offers weight-inclusive care.

Dallin Bawden has worked in the mental health field for over six years, both in outpatient and inpatient settings. He earned his nursing undergraduate degree from the University of Utah. He then earned his Doctor of Nursing Practice (DNP) graduate degree from the University of Utah in their Psychiatric Mental Health Nurse Practitioner (PMHNP) program.

Dallin has experience working with a variety of mental illnesses, including: Bipolar Disorder, Schizophrenia, Depression, Anxiety and more. Dallin has a firm belief in holistic care–he believes that there are a variety of options for treatment and care, and is dedicated to working with each of his patients to help them come up with a treatment plan that feels best for them individually. Medication management, therapy, Ketamine, and Spravato are just a few of the elements he incorporates into his practice. Dallin also provides LGBTQ+ affirming care.

Originally from Utah, Jacob spent many years living in New York, where he greatly enhanced his nursing practice. Graduating as valedictorian from the State University of New York, he gained invaluable experience in neuro intensive care at a comprehensive stroke center. His tenure there included advanced training with the palliative medicine team of the hospital, providing care to patients nearing the end of life and their families. Through these experiences, Jacob cultivated a profound empathy for the emotional rigors of illness and loss, deepening his understanding of the transformative impact of life’s transitions. Returning to Utah, he furthered his expertise in emergency and trauma medicine, as well as quality improvement and medical review spanning over 15 medical specialties. Driven by a passion for holistic care, Jacob pursued advanced education at Gonzaga University in Washington, earning his NP degree. Realizing how much psychiatric care could support people in staying well and finding their way back to wellness, Jacob decided to complete the psychiatric mental health track. His enjoyment of listening and his belief in caring for the whole person made psychiatry the right choice for him to provide personalized care to his patients. Dr. Moyers is the Medical Director of Lone Peak Psychiatry. His doctorate training was at the University of Utah’s top-ranked program in Psychiatry and Mental Health Nursing Practice where he was an R. Harold Burton Fellow. He has worked in all levels of psychiatric care over the last 15 years. His experience began as a psychiatric technician working with pediatric and adolescent patients, as a nurse with young adults in addiction recovery, and then as a charge nurse at the Utah State Hospital pediatric unit. Doctorate-level internships were completed at Polizzi Clinic, the Utah State Hospital, the VA Hospital, and West Ridge Academy.

As a nurse he has worked with forensic, adult, adolescent, and pediatric populations. Three of those years were as a nurse working directly with children with a variety of presentations including behavioral disorders, fetal alcohol syndrome, autism spectrum and pervasive developmental disorders, and psychosis.

Dr. Moyers has spent the last 5 years working with adult and pediatric patients providing medication management, psychotherapy, and interventional psychiatric treatments including ketamine, Spravato, and Transcranial Magnetic Stimulation (TMS).
